Satélite Familiar's Family Nurse at home service was created for people who need regular clinical follow-up, health monitoring and personalised nursing care, without having to travel frequently to hospitals, clinics or health centres.

Over 30 days, the client benefits from a weekly nursing visit at home, with time to assess, care, answer questions and follow their progress.

More than performing one-off procedures, the Family Nurse gets to know the clinical situation, identifies needs and helps organise an individualised care plan, in coordination with the client, the family, carers and, when needed, other health professionals.

The goal is to promote greater safety, comfort, autonomy and quality of life at home.

What is the Family Nurse service?

The Family Nurse is a continued follow-up service, designed to meet each person's health needs in a close and organised way.

Throughout the month, the nurse follows the client's progress, monitors their health, provides the necessary care and guides the family on warning signs, medication, prevention of complications and daily care.

This regular follow-up makes it possible to get to know the person better, anticipate needs and adapt the plan whenever there are changes in their clinical or functional condition.
