Essential support so you can be recovering at home, sooner.

If your family member is or has been hospitalized and is being discharged, Satélite Familiar helps prepare everything to receive them home — providing comfort, safety and mobility.

Preparing the Home

We adapt the home so your family member can return safely and comfortably:

  • Adapt the bedroom: bed and mattress
  • Prepare the bathtub or shower
  • Adapt the toilet for easier sitting and standing
  • Indoor mobility: wheelchair or walker
  • Provide rest and comfort outside the bed: recliner
  • Other adjustments as needed

Discharge Documentation

At hospital discharge, the patient should be accompanied, whenever possible, by the following documents:

  • Medical discharge note — clinical situation and current medication
  • Nursing discharge note — assessment of nursing care needs
  • Social Services notes
  • Notes from the nutritionist, clinical psychologist, and others
  • Records of training and education provided to the patient and informal caregivers
  • Copies of complementary diagnostic exams or others
  • Information on upcoming scheduled consultations or exams, with the contact responsible for follow-up
